Set appPool timeout in Azure service definition

This patch adds a new file, /src/Orchard.Azure/Orchard.Azure.Web/startup/ConfigureIIS.cmd , and adds a <StartUp> step in Orchard.Azure.CloudService/ServiceDefinition.csdef to execute the new ConfigureIIS.cmd.

ConfigureIIS.cmd sets the appPool "idle timeout" setting to 0 (never shut down the app pool due to being idle).

This should improve app pool restart issues when running Orchard in Azure.
