Hard to say with that little information. Doesn't seem directly related to Orchard however (it has nothing to do with COM). It looks more like bad IIS configuration. Is this configured for integrated pipeline, full trust?

IIS Running on Server 2012.
DotNet 4.5 is installed on the servers in question.
Integrated pipeline
Full trust

IIS Configuration is shared across multiple servers. All servers exhibit the same problem. Certainly could be a configuration issue.

No other websites or applications in the farm exhibit this behavior.

This is the only site that uses Orchard. Does Orchard have prerequisites that need to be installed that we might be missing?

What else should we check?

.NET 4, IIS 7 integrated pipeline, full trust are the minimum requirements. That is all.
