Custom Forms and Boolean Field

I created a boolean checkbox for the user to accept terms of service. I want to make the checkbox required. However, that option is not available with Custom Forms (in the Type Definition area). I am guessing this is not possible?

I then thought to create an enumeration checkbox with one choice and make it required. The problem I need to obviously get rid of the <label> for the checkbox, which would affect all other enumeration fields. So I created an alternate for that specific field in my theme: EditorTemplate-MyForm-MyField.cshtml through Shape Tracing. That was just a blank template and I wasn't sure how to proceed.

I then thought to manually make an alernate of Enumeration.Edit.cshtml that I have in my theme (appending -MyForm-MyField to it), but that didn't work.

Any thoughts?
