Cache across a web farm

Cached Data in a web farm can be stale with no way of invalidating.  Consider a multi-node web farm and a multi-data center environment where the farm spans across multiple physical locations.  

Some modules may be written to invalidate cache.  For example, on a Navigation module, adding or removing a new menu item will modify the database, then refresh cache in memory.  The problem is, on a multi-node web farm, the cache is NOT updated, and if there's no time component, never will be unless the apppool is reset.
This needs to be thought out and discussed in context with NHibernate / Microsoft AppFabric.
